Do You Have a EPC/CDM Referral from Your GP?
Chronic Disease Management (CDM/EPC) Referrals from your GP allow you to claim money back from Medicare for PART of your visit to a MAXIMUM of 5 visits in a 12 month period from the date of referral. Your doctor needs to identify a chronic issue (lasting longer than 6 months) and complete relevant paperwork/billing to enable this. If you provide us with a copy of your VALID referral when you attend we can submit this on your behalf.
